    Metrocare is the largest provider of mental health services in Dallas County, serving more than 57,000 adults and children annually. For more than fifty (50) years, Metrocare has provided an array of services available to people with mental and developmental disabilities. In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research we are advancing mental health beyond Dallas County.

     

    The Housing QMHP provides supportive housing case management services to individuals with mental illnesses who have experienced homelessness or instability in housing. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention, life skills training, tenancy skills training, and continuity of care. This position promotes ongoing housing stability, promotes the acquisition of income to increase self-sufficiency, and encourages self-determination of program participants.

    Responsibilities

    ESSENTIAL DUTIES AND RESPONSIBILITIES: 

    The essential functions listed here are representative of those that must be met to successfully perform the job.

    • Provides overall service coordination, referral, linkage, and advocacy to consumers with varying needs. Performs duties in concert with other members of an interdisciplinary treatment team.
    • Completes ongoing psychosocial assessments for program participants. Upon assessment, formulates, collaboratively an individual housing service plan.
    • Develops measurable objectives and goals agreed upon by the program participant.
    • Assists in obtaining and modifying goals as needed.
    • Provides skills training to promote stability in housing (budgeting, self-care, developing natural supports, and independent/instrumental living skills).
    • Documents attempts to involve program participants, service providers, natural supports, and caregivers (unless opposed by the program participant) in the housing service plan process.
    • Participates in interdisciplinary team staffing. Present psychosocial assessment findings. Updates team on progress or lack of progress in reaching agreed upon goals. Provides relevant information that might impact housing stability and treatment engagement.
    • Follows up with assigned program participants who miss a scheduled clinic appointment and document attempts to engage with program participant.
    • Reports repeated unsuccessful efforts in maintaining contact with program participants.
    • Follows up within 24-hours on assigned program participants who access emergency services. Report status disposition to treatment team.
    • Provides crisis intervention. Makes home visits, initiate mental illness warrants, and work with police and other public servants as needed to address crisis.
    • Facilitates inpatient admission upon request by program participant or if clinically indicated. Provides continuity of services throughout inpatient stay. Participates actively in discharge planning. Meets with program participant within 5 days of discharge into the community.
    • Conducts individual and group rehabilitation meetings. Solicits feedback from program participants, and consults with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
    • Identifies and assists program participants in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
    • Assists housing operations in obtaining documentation to complete housing recertification to facilitate ongoing access to housing assistance by program participants.
    • Performs other duties as assigned.

    Qualifications

    EDUCATION AND EXPERIENCE:

    Bachelors Degree or Masters degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention; or is a registered nurse.

    REASONING ABILITY:

    COMPUTER SKILLS:

    Intermediate level on MS Excel, Email and word processing programs
    Ability to utilize Internet for resources.

    TRAVEL:

    In-county travel.
    No overnight travel required.

    Positions that are “community-based,” in whole or part, require the incumbent to travel between various worksites within his/her workday/workweek. The incumbent is required to have a valid drivers’ license and reliable transportation. The incumbent is further required to meet the criteria for insurability by the Center’s risk management facilitator; and produce proof of minimal auto liability coverage when applicable. Failure to meet these terms may result in disciplinary action up to and including termination of employment, contract or other status with Metrocare.
