    Metrocare is the largest provider of mental health services in Dallas County, serving more than 57,000 adults and children annually. For more than fifty (50) years, Metrocare has provided an array of services available to people with mental and developmental disabilities. In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research  we are advancing mental health beyond Dallas County.

     

    The Options Counselor role is to perform activities that facilitate access to community supports, services and benefits.  The Options Counselor shall perform options counseling, a person-centered, interactive and decision-support process, and application assistance on benefits for state and federal programs through the Aging and Disability Resource Center (ADRC). 

    Responsibilities

     

    The essential functions listed here are representative of those that must be met to successfully perform the job.

    • Serves as the primary point of entry for all consumers accessing the ADRC
    • Provide information and counsel to the general public, particularly elderly adults and individuals with disabilities, with information and assistance to a range of community resources and help inform and educate people about their options and benefits
    • Coordinates and assists consumers with public benefits and services
    • Coordinates access to a wide range of community-based long-term services and supports
      Serves as an advocate in guiding consumers through a complex and fragmented service delivery system and assist in expediting the delivery of services
    • Performs person centered comprehensive needs assessments, including mental, physical, functional, cultural, financial, environmental, and life goals to determine appropriate referrals
    • Provides clinical eligibility counseling and financial prescreening for State and Federal Programs by completing the Long Term Services and Support (LTSS) screen as implemented by the State
    • Develops preliminary care planning, long term support counseling, conducts information gathering and provides limited follow-up based on consumer preferences
    • Assures all client records are maintained by following the State LTSS screening documentation policies and standards
    •  Uses telephone skills and interviewing techniques using active listening skills over the phone, or in person
    • Gathers sufficient information to accurately identify client’s problems and needs, explores needs beyond the presenting problem, looking at basic needs first and long term needs and solutions, and summarizing what they are requesting
    • Searches through resource database, as well as other written and computer-based information resources to evaluate and suggest potential programs and services
    • Provides advocacy and/or researches additional or alternative resources (if appropriate)
    • Helps consumer get connected to appropriate services by providing individual counseling / assistance services, information and referral
    • Provides short-term case management according to ADRC service scope
    • Provides follow up as needed to determine outcomes and additional assistance in locating resources and arranging services
    • Provides information about long-term care and assists consumers throughout the eligibility determination and enrollment process and public benefit programs, if needed
    • Participates in education and outreach activities including representing the ADRC at public information fairs
    • Develop, compile, and/or distribute literature on local, state and federal benefits, eligibility requirements, and availability of resources
    • Enters accurate and complete documentation into necessary data systems
    • Monitors outreach and educational efforts and develop monthly reports for the ADRC Leadership Team
    • Collect and maintain accurate data on outreach and education efforts
    • Assist the ADRC Leadership Team in completing required project status reports for presentation to the State, Advisory Committee, stakeholders and the community at large
    • Provides reporting to ADRC program on grant activities by maintaining appropriate documentation
    • Arranges work schedule as necessary or directed to meet the program and consumer service needs
    • Completes special projects and other assignments as instructed
    • Participates in staff meetings and training activities
    • Participates and maintains all training certification, as required
    • Perform other duties as assigned

    Qualifications

    EDUCATION AND EXPERIENCE:

    • Bachelor’s degree in social services, nursing, gerontology or related social services field. Bachelor’s Social Work preferred
    • Two (2) years’ experience working in human services
    • Bilingual in Spanish preferred.
    • Related field experience in programs serving the elderly and disabled, telephone counseling, and case management.
    • Experience working with the aging and disability population
    • Knowledge of and sensitivity to aging and disability issues
    • Experience and working knowledge of public, federal and private benefits including and not limited to Medicaid, Medicare, NorthSTAR, Social Security Disability Insurance (SSDI) and Supplemental Security Insurance (SSI)
    • Experience in public speaking and able to effectively communicate information and respond to questions from professionals, consumers, caregivers, and the general public 

    MATHEMATICAL SKILLS:

    • Ability to calculate moderately complex figures to accurately report activities

    COMPUTER SKILLS:

    • Intermediate level on MS Word and Excel.
    • Email and word processing programs.
    • Ability to utilize Internet for resources.

    CERTIFICATIONS, LICENSES, REGISTRATIONS:

    • AIRS Certified Information Specialist in Aging and Disability (CIRS-A/D) (Required within 1 year of employment)
    • Benefits Counseling Level 1 (Required within 1 year of employment)
    • Licensed Baccalaureate Social Worker (LBSW) (preferred)

    TRAVEL:
     Some in-county travel may be required (</= 10%).
     Overnight travel may be required (</= 10%).
