    Metrocare is the largest provider of mental health services in Dallas County, serving more than 57,000 adults and children annually. For more than fifty (50) years, Metrocare has provided an array of services available to people with mental and developmental disabilities. In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research we are advancing mental health beyond Dallas County.

     

    The Veteran Service Case Manager provides psychosocial/rehabilitative service to persons with mental illness. Services may include, but are not limited to, assessment, development and measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention, and continuity of care. Provide rehab/skills training in an individual or group setting. This position maximizes the consumer’s potential level of functioning, reduce hospitalization, and aid in the successful reintegration of consumers into the community.

    Responsibilities

    The essential functions listed here are representative of those that must be met to successfully perform the job.

    • Provides overall service coordination, referral, linkage, and advocacy to consumers with varying needs. Performs duties in concert with other members of an interdisciplinary treatment team.
    • Completes a psychosocial assessment on each new admission or upon assessment as clinically indicated. 100% compliance expected.
    • Formulates individual service plan based on assessment findings on all new admission. 100% compliance expected.
    • Develops measurable objectives and goals agreed upon by the consumer.
      • Assists in obtaining and modifying goals as needed.
      • Documents attempts to involve consumer, service providers, and caregivers (unless opposed by the consumer) in service plan process.
    • Reviews service plans every three (3) months or as clinically indicated. 100% compliance expected.
    • Participates in interdisciplinary team staffing. Presents psychosocial assessment findings. Encourages consumer to attend staffing. Updates team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might affect treatment.
    • Follows-up on assigned consumers who miss a scheduled clinic appointment. Documents attempts to contact consumer. Reports repeated unsuccessful efforts to contact to the treatment team.
    • Follows-up within twenty-four (24) hours on assigned consumers who access emergency services. Reports status-disposition to treatment team. 100% compliance expected.
    • Provides crisis intervention. Makes home visits, initiate mental illness warrants, works with police and other public servants as a needed to address crisis.
    • Facilitates inpatient admission upon request of the treatment team. Provides continuity of services throughout inpatient stay. Participates actively in discharge planning. Meet with the consumer within 5 days of discharge into the community.
    • Provides rehabilitation sessions in an individual, group or family session. Evaluate progress of rehab/skills training, solicit feedback from consumer(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
    • Co-facilitates family education workshops.
    • Identifies and assists consumers in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
    • Performs other duties as assigned.

    • Knowledge of military culture and veteran and family needs.
    • Knowledge of North Texas VA Healthcare system.
    • Ability to organize work flow and develop efficient clinic procedures.
    • Ability to effectively manage a large caseload of consumers with varying needs.
    • Ability to problem solve, exercise good judgment, and make sound decisions.
    • Skilled in using tact and diplomacy in interacting with staff and consumers.
    • Ability to work as a team member and independently with minimal supervision.
    • Effective written and verbal communication skills.
    • Ability to organize and prioritize tasks.
    • Able to work flexible hours. 

    Qualifications

    EDUCATION AND EXPERIENCE:            

    • Bachelor’s degree in human services with one (1) to three (3) years’ experience in MH/MR or related field or Master’s degree with zero (0) to two (2) years’ experience in human services.
    • Military service background preferred.

    MATHEMATICAL SKILLS: 

    • Basic math skills required.
    • Ability to work with reports and numbers
    • REASONING ABILITY:
    • Ability to apply common sense understanding to carry out simple one or two-step instructions.
    • Ability to deal with standardized situations with only occasional or no variables.

    COMPUTER SKILLS: 

    • Use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, and PowerPoint).
    • Ability to successfully use an automated clinical record keeping system.
    • Ability to utilize Internet for resources.

    CERTIFICATIONS, LICENSES, TRAINING, REGISTRATIONS: 

    • Current State of Texas Driver License, or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
    • Liability insurance required if employee will operation personal vehicle on Center property or for Center business. Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
