Metrocare is the largest provider of mental health services in Dallas County, serving more than 52,000 adults and children annually. For more than fifty (50) years, Metrocare has provided an array of services available to people with mental and developmental disabilities. In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research and our software initiative, XenatiX, we are advancing mental health beyond Dallas County.

 

To promote quality of care for treatment team patients. To provide clinical leadership for multi-disciplinary team activities that stabilize psychiatric symptomatology and maximize community functioning for DMS patients. To provide and document psychiatric evaluation and treatment to assigned patients. To promote a consumer-centered environment conducive to effective teamwork, good staff morale, and valuing of the strengths/resources of patients and other staff.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES: (Other duties may be assigned.)

  • Therapeutic Relationships:
    • Establishes and maintains therapeutic relationships with assigned patients and, as appropriate, with their natural support system.
    • Communications with assigned patients and significant others reflect that their opinions, feelings, and participation are valued as active partners in the treatment/rehabilitation process.
    • Endeavors, within existing resources, to see each scheduled patients on time, and spends adequate time to hear their concerns. Returns phone calls on a timely basis.
    • Responds to questions from patients receiving service and/or their families directly and proactively. Provides information (in language appropriate to their level of functioning) to assist in understanding and managing symptoms of mental illness.
    • Sets appropriate therapeutic limits with each patient receiving service, as needed, and explains the rationale clearly to the patient, family and other team members.
    • Reinforces each patient’s therapeutic relationship with other team members. Role models effective communication and proactive interface based on the individual’s changing needs.
  • Psychiatric Evaluation/Diagnosis:
    • Conducts and documents psychiatric evaluations in accord with procedures.
    • Takes into account all relevant data sources, including but not limited to the individual receiving service, family/significant others, observations of current and past treatment providers and review of records in conducting evaluations and formulating diagnoses.
    • Accurately applies criteria from DSM in formulating diagnoses.
    • Responds flexibly to handle unscheduled evaluations of individuals who walk in and whose level of functioning appears to indicate an imminent crisis or immediate need.
  • Treatment Planning/Implementation:
  • Takes an active role in interdisciplinary team meetings to discuss evaluations and diagnostic formulations and to assist the team and the patient to develop an appropriate plan of service which:
    • clearly states problems
    • indicates goals, measurable objectives and intervention strategies to address each identified problem
    • identifies staff responsible for each planned intervention within a specified time frame.
  • Participates in the development and updating of the individual service plans.  
  • Demonstrates good professional judgment in implementing psychiatric interventions as defined in the individual service plan, in reviewing the case with the team as needed, and in modifying the individual service plan as appropriate, based on changing needs of the individual receiving services.
  • Assures that psychiatric interventions are coordinated with the interventions of other professionals, and that other providers are kept informed of changes in the psychiatric condition/status of the individual receiving services.
  • Documents that the rationale for all psychiatric interventions, including any changes, has been explained to the individual and that informed consent has been obtained according to DMS procedures.
  • Medications:
    • Demonstrates good professional judgment in prescribing, monitoring, and adjusting psychoactive medications for each assigned patient.
    • Implements and documents medication-related practices in accord with DMS administrative procedures.
    • Documents that laboratory studies and/or physical examinations have been ordered, completed, and reviewed per DMS procedure, including prior to beginning psychoactive medications and within recommended frequencies for each medication during the course of treatment. Also documents that appropriate and timely adjustments in medications are made based on laboratory/examination findings.
    • Establishes frequency of face-to-face appointments with the physician, based on needs of each individual patient, but not less than every three months. Uses the skills of RNs and other team professionals in monitoring/assessing progress and stability of individuals receiving services.
    • Documents provision of medication education to each individual according to DMS procedures.
    • Assures that psychiatric and medical care are coordinated with the care of other community medical providers who may also be treating the individual.
    • Assures that other providers in 24-hour residential services and/or home caregivers assisting with medications monitoring are informed on a timely basis regarding medication changes, behavioral signs which need to be observed and reported, and of how and to whom to report.
    • Prescribes only medications on the DMS formulary, at doses within acceptable ranges as identified in the PDR, with exceptions to be approved by the Center Medical Director.
  • Crisis/Emergency:
    • Is responsive to psychiatric and/or medical emergency care needs of individuals receiving services.
    • Maintains current knowledge of Texas Mental Illness Code and necessary procedures for involuntary hospitalization.
    • Works closely with other staff in determining when admission to a more restrictive setting is indicated.
    • Accurately completes necessary documentation for obtaining an Order of Protective Custody, when applicable.
  • Psychotherapy:
  • As time and resources permit, may conduct short term, goal-oriented individual or group psychotherapy with assigned individuals, with appropriate documentation.
  •  
  • Documentation:
    • Documentation in individual service records will be sufficient for filing and justification of insurance/third-party claims for the service provided.  
    • Documentation of time will be in accord with procedures for every service event provided
    • Adheres to DMS procedures in maintaining security of confidential consumer information.  
    • Completes Unusual Incident Reports as appropriate and according to administrative procedures.
  • Quality Improvement:
    • Schedules sufficient percentage of time for the provision of face-to-face interactions with consumers to achieve productivity benchmarks assigned.
    • Documents these face-to-face interactions in the medical record in a manner consonant with DMS standards for quality of documentation.
    • Provides clinical care in a manner consonant with standards, and review of medical records demonstrates documentation of psychiatric evaluation, treatment planning and medication prescription consonant with DMS quality of care standards.
    • Actively participates in unit and DMS continuous quality improvement initiatives, including utilization review, review of cost efficiency, review of productivity benchmarks, and peer review.
  • Communications:  
  • Demonstrates good professional judgment in keeping supervisors and unit coordinators informed on a timely basis of any pending problems in service delivery or other role related issues.
  • Models open and constructive communication to identify and resolve service delivery problems. Works collaboratively with the Unit Medical Director toward resolution of complaints related to physician and treatment team care.
  • Keeps the Unit Medical Director and other administrative supervisors informed of any community contacts which may have the potential to affect Metrocare Service public image and of media/publicity requests requiring a response.
  • Immediately reports to the Center’s Consumer Rights Protection Officer and to Unit Medical Director any instance of consumer, family or community provider complaints; dissatisfaction with services; suspected abuse and/or neglect; or any other violation of rights.
  • Supervision:
  • Meets with Unit Medical Director on a regular basis and utilizes good professional judgment in seeking consultation between regular supervisory meetings. Utilizes supervisory conferences constructively and demonstrates follow-through on needed action or performance improvements identified in supervision.
  • Regularly attends medical staff meetings scheduled by the Unit Medical Director.  
  • Training/Professional Development:
  • Stays current with trends in psychiatry and shares information with other medical and non-medical staff.
  • Remains current on all DMS mandatory training.
  • Remains current on Texas State Board of Medical Examiner continuing education requirements.
  • Takes advantage of opportunities for professional development and arranges through Unit Medical Director to attend job-relevant continuing education activities.
  • Prepares and presents structured training to other staff in areas of particular expertise upon request.
  • Scheduling:
  • Adheres to assigned work schedule and gives prior notification to Unit Medical Director regarding any need to be absent, or to otherwise deviate from work schedule.
  • Maintains satisfactory attendance and punctuality.
  • Submits personal leave requests for vacation of up to three (3) consecutive days at least two (2) weeks in advance. Vacation requests of more than three (3) consecutive days will be submitted at least thirty (30) days in advance in order to address needs of client caseload with adequate planning. Changes in scheduling are made only after written leave requests are approved.  
  • In any instance of planned leave, anticipates and coordinates planning with other unit psychiatrists for how caseload needs are to be addressed.

Qualifications

EDUCATION AND EXPERIENCE:            

Graduate from an accredited Medical School and completion of an approved residency in Psychiatry. During residency training or subsequently, clinical experience in community mental health treatment modalities, psychiatric rehabilitation program concepts, crisis intervention techniques, assessment and differential diagnosis.

 

REASONING ABILITY:

  • Ability to work collegially with other members of the interdisciplinary team.
  • Ability to articulate and synthesize information gained from a variety of assessments into an individual service plan.
  • Ability to implement psychiatric interventions in concert with other psychosocial interventions.  

COMPUTER SKILLS: 

  • Intermediate level on MS Excel, Email and word processing programs
  • Ability to utilize Internet for resources.

CERTIFICATIONS, LICENSES, TRAINING, REGISTRATIONS:

  • Licensed to practice medicine in Texas.
  • Current DEA and DPS registration for the prescription of controlled substances
  • CPR
  • PMAB
